TERMS & CONDITIONS

REGISTRATION

Sign up at your child’s school. Registration and payment are expected by the first class. We must have five students registered to schedule the first class. Complete and sign the registration form. Please hand the form to the Instructor. If the instructor is not available, you can put the form and payment in MIKIO SURAI / Myloreal's Martial Arts Academy L.L.C. Payment Box. If the box is full, mail in the form with the registration fee and monthly tuition. You may also choose to register online or download the registration form, sign and scan it, and email it to mylorealsmma@gmail.com.

PAYMENTS & ACCOUNTS

DO NOT LEAVE THE PAYMENT WITH YOUR CHILD'S TEACHER. If you leave tuition payment with a teacher, MIKIO SURAI / Myloreal's Martial Arts Academy L.L.C. will not be responsible for its loss. All checks returned by the bank will be charged a $35.00 NSF fee. After one returned check, paying online, money orders, or cash must be the method of payment. You will have 5 days from the date you are notified to submit the payment in full. If the payment in not received within this time frame, the account will be closed, the child/ren will not be able to participate, and you will have to repay the fee of $99.00 in addition to your tuition and NSF fee of $35.00.

Students whose accounts are not paid by the 15th of the month will not be able to participate. MIKIO SURAI / Myloreal's Martial Arts Academy L.L.C. does not prorate or provide make-up classes for missed classes due to your child’s absence, inclement weather, school closures, or holidays. In the event your instructor misses a class, a make-up class will be scheduled.

Once your tuition payment has been made, NO REFUNDS will be given. We accept checks, cash, money orders, or online payments via PayPal.Statements will be sent out each month as a courtesy reminder. Payments are expected with or without receipt of a statement. All payments are final. There are no refunds given under any circumstances. In order to keep the school up and running, your monthly tuition MUST be paid on time. All tuition payments are due within the first week of the new month.Whether you attend every available class in a month or you do not attend any classes within the month, your tuition is still due as long as you are a student of MIKIO SURAI Martial Arts International. Unlike other martial arts programs, we do not have our students sign a yearly contract.

WITHDRAWAL & DISMISSAL

Withdrawal notification must be made in writing via email to mylorealsmma@gmail.com or mailed to P.O. Box 1727 Mableton, GA 30126 prior to the first day of a new month or tuition will be due.For the benefit of the class, safety, and avoiding injury, certain behavior is expected of all students. If all efforts to satisfy this goal fails, MIKIO SURAI / Myloreal's Martial Arts Academy L.L.C. reserves the right to withdraw a student from the class. If this action happens there will be NO REFUND given. In the event that 3 children have not registered for the class and you have paid your tuition, you will be offered private lessons totaling the value of your tuition payment.

DISCOUNTS

For Discounted Students: If you are a student that receives a discount, you will lose your discount if your tuition is not paid within the first week of the new month. If you lose your discount due to nonpayment, you will have to pay the regular tuition. You will be allowed a one-week grace period if needed, but not at the discounted rate. If payment is not received by the 15th of the month, there will be a $10.00 late fee added to your tuition. 

For Non-Discounted Students: Payment is due within the first week of the new month. You will be allowed a one-week grace period if needed. If payment is not received by the 15th of the month, there will be a $10.00 late fee added to your tuition.

All Students: If you are more than 21 days past due on your tuition, you may not take classes until your tuition is paid in full.If your membership goes unpaid for more than 28 days, you will forfeit your membership to the school, your discounted rate (if any), and your initial sign up fee. If you want to attend after you have forfeited your membership, you will need to repay the sign up fees and the past due tuition.

Discounts are only given ONE TIME. If you stop, quit, or take a break, you must still pay the discounted tuition. If you do not continue to pay the discounted tuition and decide to return after taking a break, another fee will be added and you will pay the regular tuition rate with no discount. Discounts can NOT be combined with other discounts.
